Dental Chart
Dental Chart keeps one chart per patient with two kinds of entries, each recorded in exactly one place:
- Findings: what you found on a tooth. Either a problem (Caries, Fracture, Needs extraction, Missing, Watch) or existing work done at another clinic (Filling, Crown, Root canal, Implant, Bridge, Veneer, Sealant). Findings are recorded only on the chart.
- Treatments: your clinic’s own work. A patient treatment done on teeth carries each tooth, its surfaces, and the findings it resolves.
Nothing is copied between them. When a filling on 36 MO is completed, the chart draws the filling from the treatment itself, and the caries it was linked to is resolved. Notes and forms don’t hold tooth data.
You can record findings from three places, each with the context filled in for you:
- the patient’s Dental chart tab,
- the Teeth section of a treatment, which links the finding to that treatment,
- the Teeth section of an appointment, which links the finding to that visit.
Permissions
Section titled “Permissions”| Permission | Allows | Starts on these roles |
|---|---|---|
| Read dental charts | Open the Dental chart tab and the Teeth sections. | Doctor, Nurse, Manager |
| Update dental charts | Record findings anywhere, and resolve or remove them. | Doctor, Nurse |
Anyone with Read dental charts who can edit a treatment (it’s assigned to them, or they have Update patient treatments) can also record findings from that treatment’s Teeth section, while the treatment is still open. You can always undo your own new findings for 15 minutes. Picking teeth for a treatment follows the treatment’s permissions.
Teeth and numbering
Section titled “Teeth and numbering”The chart shows the 32 permanent teeth, Upper and Lower, laid out as you face the patient: the patient’s right is on your left (Right · Left). Select Baby teeth to also show the 20 primary teeth, as two smaller rows between the permanent ones. A chart that already has baby teeth recorded shows them automatically.
Your clinic chooses how teeth are numbered, in Dental chart settings. The same numbers are used everywhere: the chart, treatments, invoices, printouts, reports and the AI Assistant.
| Numbering | Permanent teeth | Baby teeth | Upper right first molar | Upper right baby second molar |
|---|---|---|---|---|
| FDI (the default) | 11-18, 21-28, 31-38, 41-48: the quarter of the mouth, then the tooth counting out from the middle | 51-55, 61-65, 71-75, 81-85 | 16 | 55 |
| Universal | 1-32, from the upper right wisdom tooth round to the lower right one | A-T, in the same order | 3 | A |
| Palmer | UR, UL, LL or LR for the quarter, then 1-8 counting out from the middle | the quarter, then A-E | UR6 | URE |
Hover a tooth to see its name.
Dental chart settings
Section titled “Dental chart settings”With settings access, open Settings and select Dental chart (“How teeth are numbered and named everywhere in the app.”):
- Tooth numbering: FDI (two digits), Universal or Palmer. Preview shows a few teeth in the chosen numbering. It changes how every tooth is shown, including on charts and invoices you open later. Invoices already issued keep the text they were issued with.
- Tooth names: every tooth comes with a name in each language, for example “Upper right first molar”. Pick the language at the top, then Permanent or Baby teeth, and type your own name for any tooth. A tooth you leave empty uses the default name. Use the default name next to a tooth, or Reset for a whole language, goes back to the defaults.
Select Save changes. You see the new numbering and names straight away; other staff see them the next time they open or reload the app.
Record a finding
Section titled “Record a finding”Tapping a tooth never records anything. It only shows the tooth. To record, select Record finding. The Record a finding window walks you through it, and nothing is saved until you select Save finding:
- Teeth: the teeth you had selected are already there. Remove one with ×, or Choose on chart to add more. Every selected tooth gets the same finding.
- What did you find?: pick one card, under Problem (Caries, Fracture, Needs extraction, Missing, Watch) or Existing work (done elsewhere) (Filling, Crown, Root canal, Implant, Bridge, Veneer, Sealant). Each card says what it means.
- Surfaces: only for caries, fractures, fillings, veneers and sealants. Tap the surfaces on the drawing or by name: B buccal, L lingual, M mesial, D distal, and O occlusal or I incisal in the middle. Leave them empty when it affects the whole tooth.
- Visit and note: the patient’s visit that is in progress (checked in or in the chair) is chosen for you. Pick another of their recent visits or Not during a visit, and add a note if needed. From an appointment, the visit is that appointment.
The bottom of the window says exactly what will be saved, for example “Caries MO on tooth 36”. Select Save finding, or Save and add another to keep the same teeth and record the next finding.
A message confirms what was saved, with Undo and how long undo stays available. You can undo your own findings for 15 minutes from when you saved them. Until then, each of your new findings shows Undo with the time left counting down, for example Undo · 12:48. When it reaches zero, the button goes away and you Resolve or Remove the finding instead. Other staff never see Undo on your findings.
Each finding then shows Found during the treatment and Found in the visit of a date, with links.
A patient’s dental chart
Section titled “A patient’s dental chart”Open the patient and select the Dental chart tab (“Everything recorded on this patient’s teeth: findings, treatments and their history.”). Everything on the patient’s teeth is on one page, so you can keep it open while the patient is in the chair.
Summary
Section titled “Summary”The cards at the top count Open findings (and how many teeth they’re on), Missing teeth, Planned treatments and Completed treatments.
The chart
Section titled “The chart”Each tooth is drawn with its crown, divided into its five surfaces, and its roots. It shows the open findings plus the work your clinic completed, drawn from the treatments. The chart uses only a few colours, each with one meaning:
| On the chart | Means |
|---|---|
| Red | A problem: caries or a fracture on its surfaces, or a red cross for Needs extraction. |
| Blue | Existing or completed work: fillings, sealants and veneers on their surfaces, a thick outline for a crown, the roots for a root canal, a post for an implant, a bar for a bridge. |
| Amber dashed outline | Watch: check again at the next visit. |
| Faded and crossed out | Missing, unless an implant or bridge replaced it. |
| Dot under the number | A treatment planned or in progress on that tooth. |
The legend under the chart lists only what this patient’s chart shows. Planned treatments aren’t drawn as work yet: the tooth keeps showing the problem (for example the red caries) until the treatment is completed.
Hover a tooth for a one-line summary. Tap a tooth to see everything about it. The Treatments switch (on by default, remembered by your browser) shows or hides treatment dots, the treatments on each tooth and treatment events in the history.
Teeth at a glance
Section titled “Teeth at a glance”With no tooth selected, Teeth at a glance lists every tooth that has something on it, teeth with open findings and planned work first. Each row shows the tooth’s findings, then its treatments by name with their state: the status while planned or in progress (a dot in the same colour as the chart’s planned-treatment dot), or Done and the date once completed (a blue dot, like completed work on the chart).
Treatments done on many teeth at once, such as a cleaning or whitening, are listed once under Across the mouth, with how many teeth they cover, instead of repeating on every tooth. Select a row, or a tooth on the chart, to see it in full.
Tooth details
Section titled “Tooth details”Select one or more teeth on the chart, or use Select upper and Select lower. Record finding then starts with these teeth, and Plan treatment lists your catalog treatments (those that resolve what’s on these teeth first). Each selected tooth gets a card with:
- Current findings, with who recorded them, when, and where they were found,
- Treatments on that tooth, with their status, plan, clinician and dates. Select one to open it,
- its resolved findings, under resolved findings.
Select Clear to go back to the overview.
History
Section titled “History”History lists everything that happened on the teeth, newest first and grouped by day: findings recorded, resolved and removed, and treatments added and completed. Use All, Findings and Treatments to narrow it. While teeth are selected, it shows only their history.
Plan a treatment from a finding
Section titled “Plan a treatment from a finding”Next to an open problem, select Plan. The menu lists the catalog treatments that usually resolve it, for example Dental filling for caries. Pick one and it is planned straight away, at the catalog price, on that tooth and its surfaces and linked to the finding. A message confirms it, with Open to go to the new treatment. Pick Another treatment… to fill in the full form instead, with the patient, the tooth and the finding already filled in. Once a treatment is planned for a finding, Plan no longer shows on it.
Plan treatment above the selected teeth lists the same treatments, but opens the full add-treatment form instead of planning at once. The patient, the treatment, every selected tooth and the open findings the treatment resolves are already filled in, so you check them and save.
Plan needs the Treatments module and the Create patient treatments permission.
Resolve or remove findings
Section titled “Resolve or remove findings”Each finding has one button, depending on what it is:
- Problems (Caries, Fracture, Needs extraction, Watch): Resolve, when it no longer applies and no treatment fixed it. Usually you don’t need it: completing a treatment linked to the problem resolves it for you. Plan is shown on problems only.
- Missing and existing work (Filling, Crown, Root canal, Implant, Bridge, Veneer, Sealant): Remove, after a confirmation. Use it when the work is no longer there, or when something was recorded by mistake. A missing tooth doesn’t get “resolved”: to replace it, plan an implant or a bridge, and the chart draws it from that treatment.
Either way, the finding leaves the chart but stays on the tooth’s card and in the history, as Finding resolved or Finding removed, with who did it and when.
Findings are part of the medical record. A patient with any dental chart findings, including resolved ones, can’t be deleted: “The dental chart is kept for the medical record and can’t be deleted with the patient.”
Print the chart
Section titled “Print the chart”With the Print module on, select Print at the top of the tab. The A4 printout has everything on the patient’s teeth:
- the patient, Chart as of (the date and time it was printed) and Printed by, so a later change to the chart is never confused with what is on paper,
- the summary counts and the chart itself, drawn exactly as on screen, with its legend,
- Tooth by tooth: every tooth with a finding or a treatment of its own, with its findings and treatments (with surfaces). Treatments done on much of the mouth, such as a cleaning, are listed once under Across the mouth with how many teeth they cover, as on screen,
- Current findings with surfaces, notes, who recorded them, when, and the treatment they were found during,
- Treatments with teeth and surfaces, status, plan, clinician and dates,
- Full history: every finding recorded, resolved or removed and every treatment added and completed, newest first. Treatments done on much of the mouth show how many teeth they cover instead of every tooth number.
Treatments and treatment plans print their teeth on the same chart drawing.
Teeth on treatments
Section titled “Teeth on treatments”With Treatments also on, set up each catalog treatment that is done on teeth under Clinical options in the catalog:
| Option | What it does |
|---|---|
| Done on specific teeth | Patient treatments made from it ask for Teeth. Others, such as a consultation, never show a tooth chart. |
| Needs surfaces | Each tooth must have its surfaces, for example MO for a filling. |
| Price is per tooth | The price and discount are for one tooth. The treatment’s estimate, its plan’s totals, reports and the invoice line multiply them by the number of teeth, for example 3 fillings at 120 are 360. Edit the treatment and you still see the per-tooth price. Off, the price is for the whole treatment however many teeth it covers. |
| Shows on the chart as | What the tooth is drawn with once the treatment is completed, for example Filling, Crown or Missing for an extraction. |
| Usually resolves | The findings a tooth’s open findings of these kinds are linked to automatically when you pick the tooth, for example Caries for a filling. |
When you add or edit the treatment, the Teeth input is the patient’s own chart:
- Tap a tooth. It’s added with the open findings the treatment usually resolves already linked, and their surfaces filled in. A tooth with caries on MO becomes “36 MO, resolves Caries MO” in one tap.
- Change the surfaces or tap the tooth’s findings under Resolves to link or unlink them. Tap the tooth again, or ×, to remove it.
When the treatment is Completed, exactly the linked findings are resolved, with Resolved by the treatment, and the tooth is drawn as Shows on the chart as on the treatment’s surfaces. The teeth are also:
- shown first on the treatment’s page in a compact, read-only Teeth section: a small chart picture next to the findings on those teeth. Each finding says whether it is Treated by this treatment, Found during this treatment or Resolved. At the top of the section, This treatment shows only the treatment’s teeth, and Full chart shows the patient’s whole chart (findings, completed work and a dot on every tooth with a planned treatment) with this treatment’s teeth highlighted. A line under the picture says which one you’re looking at. To add a finding, select Record finding, which links it to the treatment. An appointment’s Teeth section works the same way with This visit: the teeth of the treatments linked to the appointment (listed above the findings, with their teeth and status), the findings on those teeth, and anything recorded in the visit. Record finding starts with those treatments’ teeth,
- shown in treatment lists, for example “Teeth 36 MO, 37”,
- added to the invoice line in brackets, for example “Composite filling (36 MO, 37)”, when the treatment is invoiced.
Cancelled treatments are left off the chart.
Reports and assistant
Section titled “Reports and assistant”- The Dental chart findings data source lists every finding, open or resolved, with the tooth, quadrant, surfaces, note, who recorded and resolved it, how long it has been open, whether it was found during a treatment or in a visit, and whether a treatment resolved it. The Findings waiting for treatment template lists open caries, fractures and teeth marked for extraction, longest waiting first: patients to plan treatment with.
- The patient’s 360 view has a Dental chart section, with an alert naming the teeth that need treatment.
- The AI Assistant can read a patient’s whole chart tooth by tooth and, after your approval, record or resolve findings.
Troubleshooting
Section titled “Troubleshooting”| You see | What to do |
|---|---|
| No Dental chart tab or Teeth section | You need Read dental charts, and the module must be on. |
| No Record finding button | You need Update dental charts. On a treatment, being able to edit that open treatment is enough. |
| “You can’t record findings here. You need to be able to edit this treatment or update dental charts.” | Ask for Update dental charts, or record from a treatment you can edit. |
| “This finding can’t be undone any more. Resolve or remove it instead.” | Undo works on your own findings for 15 minutes. Use Resolve for a problem, or Remove for a missing tooth or existing work. |
| “Surfaces can only be M, O, D, B, L or I, each once.” | Pick each surface once. |
| “Pick each tooth once, from the chart.” | A treatment’s teeth must be picked on the chart, each tooth once. Pick them again. |
| “Pick the surfaces for tooth 36.” | The catalog treatment Needs surfaces. Tap the tooth’s surfaces. |
| “A finding linked to tooth 36 is no longer open. Refresh and pick again.” | Someone resolved that finding meanwhile. Reload the page and pick the tooth again. |
| No Teeth when adding a treatment | Turn on Done on specific teeth under the catalog treatment’s Clinical options. |
| “This treatment isn’t set up for this detail. Turn it on in the treatment’s clinical options first.” | The treatment isn’t set as done on specific teeth. Turn that on in the catalog, then try again. |
