Filling in and correcting forms

You need Create forms and Submit forms.

  1. Open the record the form belongs to and select Add form:
    • a patient: the Forms tab,
    • an appointment, treatment or treatment plan: Actions › Add form, or Add form in its Forms section.
  2. Add a form: “Pick the form to fill in.” Forms are grouped by category, with the number of fields. Search forms to find one. Only templates that are Available to fill in are listed. If none match: “Try a different search, or ask an administrator to add one.”
  3. Answer the questions. The record you’re filing against is shown alongside (the appointment’s date, practitioner, room and status, or the treatment’s code, estimated price and invoiced amount), so you can check you’re on the right one. Use Choose another to pick a different form.
  4. Select Submit form.

You see Form submitted. It appears in the record’s Filled Forms list.

Each problem is shown under the question:

Message Meaning
“Question is required” A required question is empty, or a required tick box isn’t ticked.
“Question must be at least 10” / “at most 300” A number is outside its limits.
“Question must be at least 5 characters” / “at most 200 characters” The text is too short or long.
“Question must be a valid date” / “must be a number” The value isn’t the right kind.
“Question is archived and no longer accepts new answers.” The template was changed while you were filling it in. Reload the form.

Filled Forms on each record shows: Form, Completed (when), Completed by, Status and Amendments. When there are none: “No forms completed yet. Consent, intake and assessment forms filled for this record appear here.”

Open one to see every answer (“Not answered” for skipped questions) and its details: Record, Completed / Completed by, Last amended / Amended by and the number of Amendments, or Voided / Voided by.

The buttons at the top of a filled form:

Button Shown when What it does
Print Print is on. The filled form, every question with its answer.
Amend You have Amend forms, it isn’t void, and it’s within its amendment window (or you also have Void forms). See Amend a form.
Void You have Void forms, and it isn’t void yet. See Void a form.

To correct a filled form, for example a wrong allergy severity. You need Amend forms.

  1. Open the filled form and select Amend.
  2. Change the answers that are wrong. “The original answers are kept. This records a correction against them.”
  3. Fill in Why is this being amended?, for example “Corrected the recorded allergy severity…”.
  4. Select Save amendment. You see Form amended.

The form shows Amended and “Amended 2 times”. The Last amendment box shows the latest reason. A form can be amended again and again.

If you see This form can no longer be amended (“Its amendment window has closed, or this form does not allow amendments.”), ask someone with Amend forms and Void forms: they can still amend it. The template’s amendment rule sets the window.

When a form shouldn’t be on the record at all, for example “Filed against the wrong patient…”. You need Void forms.

  1. Open it and select Void.
  2. Why is this being voided? is required.
  3. Select Void form. “It stays on the record, marked void, with your reason attached.”

The form shows This form was voided. A voided form can’t be amended or un-voided. If it was filed on the wrong record, fill in a new one on the right record.

With Print on, a filled form can be printed from its page. Unanswered questions print as Not answered.
