Note templates
A note template is the text a note starts with: its sections and prompts, such as Subjective, Objective, Assessment and Plan. Staff pick one when they select Add note, or add one to a note they’re writing with Insert template. Only the templates meant for that kind of record are offered: a lab order shows lab templates, not post-extraction instructions.
Templates are different from form templates: a form collects answers to set questions, while a note template is only a starting text that the clinician then edits freely.
Ready-made templates
Section titled “Ready-made templates”New clinics get a starter set, in every supported language:
| Note type | Templates |
|---|---|
| Progress note | SOAP note, Initial consultation, Follow-up visit, Procedure note, Dental examination |
| General note | Phone call, Referral, Treatment declined, Clinical information for the lab |
| Pre-treatment instructions | Before your procedure |
| Post-treatment instructions | After a tooth extraction, After a filling, After root canal treatment, After scaling and cleaning, After implant surgery |
| Treatment outcome | Treatment outcome |
Each one is set to the records it suits. For example, the visit notes are offered on appointments and the patient file, the aftercare instructions on appointments and treatments, Clinical information for the lab on lab orders, and Treatment outcome on treatments.
The instruction templates are written for patients and are Visible to the patient by default. Change, retire or delete any of them like your own. The starter set is only added while your clinic has no note templates at all, so it never comes back after you’ve changed or removed it.
Manage templates
Section titled “Manage templates”You need Manage note templates (on the Doctor and Manager roles by default). Templates are under Catalogs › Note Templates: “Ready-made notes your team can start from, in every language your clinic uses.” The list shows each Template with its description, Note type, where it’s Used on (or All records), whether it’s Shared with patient, when it was Updated, and its Status (Available or Retired). Search templates finds one by name or description.
Create or change a template
Section titled “Create or change a template”- Select New template, or select a template in the list.
- Fill in the Name and, optionally, a Description (“When to use this template”).
- Write the Template text in the same editor as notes, with headings, lists and checklists from the toolbar. Leave empty bullets or “Label: “ lines where the clinician fills in details. If AI writing help is available to you, it works here too.
- On the side, choose the Type the note will have, and under Used on tick the records it’s offered on (patient file, appointment, treatment, treatment plan, lab order; only the modules you use are listed). Leave all unticked to offer it everywhere. Then choose whether notes from it are Visible to the patient by default, and whether it’s Available.
- Select Save template. You see Template saved.
Languages
Section titled “Languages”Name, description and text can be written in each language your clinic uses. Use the language switcher at the top of the form to move between them. In a language that isn’t translated yet, the editor shows the default language’s text to start from (“Showing the … text - edit it to write this language’s version”); it’s only saved for that language once you change it. Staff see a template in their own language when it’s translated, otherwise in the clinic’s default language.
Retire or delete
Section titled “Retire or delete”- Turn off Available to retire a template: it’s no longer offered when writing a note, but stays in the list.
- Delete removes it after you confirm with Yes, delete. Notes already written from it are not affected.
