After release

When a released result was wrong, correct it instead of entering a new order. You need Void and correct released lab results and Verify and release lab results in that lab.

Open the order, choose the result and correct it with a reason. The correction creates a new version of the result. The old one is kept. Clinicians see the corrected value, and the patient sees “This result was corrected by the laboratory.” Corrections are logged (“corrected a result”) and counted in the Corrected & voided results widget.

When results must be withdrawn entirely (wrong patient, contaminated sample):

  • Void the … result? for a single test, or
  • Void lab order (“Why are these results being withdrawn?”) for the whole order.

You need Void and correct released lab results. A reason is required. Voided results are withdrawn from trends, critical lists, the portal and printouts, but kept for audit. The order shows “This lab order was voided” with the reason, who and when. If the last remaining result on an order is voided, the whole order is voided. In the portal, the patient sees “The laboratory withdrew these results. Please contact the clinic if you have questions.”

Each order has a Patient portal card:

State Meaning
Shown in the patient portal “The patient can see these results in the portal” (once released), or “The patient will see the results once they are released.”
Hidden from the patient portal “The patient won’t see this order in the portal.”

Switch it with Show in patient portal (confirm with Show in portal) or hide it again at any time. Allowed for whoever placed the order, staff with Edit lab order links, and the lab’s verifiers. Each change is logged.

In the portal, patients see My lab results: “Laboratory results your clinic has released to you”, each with its accession number, who ordered it, the release date, values, reference ranges, flags (Normal, Low, High, Very low, Very high, Abnormal) and a bar showing where the value falls in the range. They’re reminded: “Results outside the reference range don’t always mean something is wrong. Please discuss your results with your doctor.”

For any test, open previous results to see the patient’s earlier values for the same test, for trend comparison. Voided values are left out.

Staff with Manage laboratories, tests and panels see each order’s Access log: every view, file opened, print, requisition and label print, link change, fee change, portal show/hide, void and correction, with who did it and when. Views by the patient through the portal are included (“via patient portal”).
